Athletics Forms
All players must complete the three forms
listed below before they will be allowed to participate in athletics at
St. Johnsbury Academy.
Athletic Department Digest/Athletic Code--STUDENT HANDBOOK (p 36)
This section is designed to provide students and parents with information
about athletics and student activities. Please familiarize yourself
with this information so that you and your parent(s) understand and
accept responsibility for our rules and policies.
St.
J. A. requires a physical examination a minimum of every year in
order to participate in interscholastic athletics. Physical exams are
at the expense of the individual, and a record of the physical must be
on file in the Athletic Director’s office before a student is eligible
to practice or compete. If the one year time limit expires during the
season, the student will be denied participation until his/her physical
is renewed. All players must have a current sports physical form on
file with Mr. Conte before they will be allowed to participate in
practices or games. A physician's form, clearing an athlete for
participation, can be faxed to the SJA Athletic Department at 751-2117.
Physicians forms must be on the doctor's letterhead.
All
players must have a signed medical release form on file with the school
so that proper medical care can be given to an injured played when the
injured player's parents/guardians cannot be contacted. All medical release forms are kept on site by all coaches at all times.
